Resumen: This article is a critical synthesis of 12 papers included in this supplement. The set deals with the short- and long-term effects of improving nutrition in Guatemalan villages characterized by deficient diets, high rates of infection and pronounced growth retardation in the first 3 years of life. The data reviewed come from two studies carried out over two decades: the Institute of Nutrition of Central America and Panama (INCAP) longitudinal study (1966-1977) and its follow-up (1988-89). The longitudinal study included a nutrition intervention that improved the energy and nutrient intakes of women and preschool children. Its effects included improved birthweight, reduced infant mortality rates and improved growth rates in children <3 years of age. Growth rates from 3 to 7 years of age, similar to those of well-nourished children, were not affected by the intervention. The follow-up study was conducted when the subjects were 11-27 years old. Among the long-term effect found were greater stature and fat-free mass, particularly in females, improved work capacity in males and enhanced intellectual performance in both genders. The nutrition intervention did not, on the other hand, accelerate maturation during adolescence, as measured by skeletal age or age at menarche. It is concluded that improved nutrition in early childhood has important long-term effects in the adolescent and adult.(AU). Resumen: The overall objetive of both the Institute of Nutrition of Central America and Panama (INCAP) studies and the Nutrition Collaborative Research Support Program (CRSP) was to determine if marginal malnutrition affects human function. The conclusions from the two studies were consistent, notably that growth stunting occurs early in life and is accompanied by functional impairments. These consequences of early malnutrition persist later in life. The comparison of INCAP and Nutrition CRSP results also illustrates that the Guatemalan children were more malnourished and stunted than those in Kenya, Mexico or Egypt and that this greater degree of stunting occurs befor 18 months of age. Even the Atole did not bring the average size of supplemented children in Guatemala up to those of the unsupplemented chidren in the CRSP populations. It is also likely that their functional potential was not realized fully by supplementation. The CRSP studies provide evidence of multiple micronutrient deficiencies associated with poor growth and function and it is probable that these associations exist in Guatemala as well. The overall conclusion is that our attention should de directed to determining the adequacy of micronutrient status in the perinatal period and to the development of approaches that prevent early growth failure.. Resumen: División de Desarrollo Humano del INCAP está investigando el efecto de la desnutrición moderada sobre el crecimiento físico y el desarrollo mental del niño, desde la concepción hasta los siete años de edad, en cuatro comunidades rurales de Guatemala. Los habitantes de dos de las comunidades reciben un suplemento proteicocalórico llamado "atole", en tanto que los de las otras dos reciben un suplemento calórico llamado "fresco". Se demostró el efecto de la suplementación calórica durante el embarazo sobre el peso del niño al nacer. Esto representa importantes implicaciones de salud pública, por la posibilidad que entraña de disminuir las tasas de mortalidad infantil y la proporción de niños con desarrollo mental retrasado en muchas comunidades pobres del mundo. Además, los datos disponibles en niños hasta los siete años de edad indican que la suplementación alimentaria ejerce un efecto poderoso sobre el crecimiento físico. Con respecto al desarrollo mental, resultó evidente que en las niñas existe una relación muy consistente entre la talla y el rendimiento en las pruebas psicológicas de vocabulario y punteo sumario, en todas las edades. En los niños estas relaciones son similares a las de las niñas a los tres, cuatro y cinco años de edad pero, con la excepción del vocabulario, casi no hay relación a los seis y siete años. Por último, la suplementación alimentaria mostró una asociación inversa con el intervalo entre partos, la cual se debe al acortamiento producido en el período de amenorrea posparto y el período de menstruación sin embarazo. Por las razones apuntadas, se postula que la desnutrición crónica a lo largo del ciclo vital afecta el crecimiento físico, el desarrollo mental y la fertilidad, produciendo efectos que podrían ser irreversibles y perpetuarse de una a otra generación.(AU).
